Our team will provide you with up-front, transparent pricing on all services, so you know exactly how much they cost. All additional fee services will be discussed with you before treatment, so there are no unexpected bills at the end of your visit.

Please remember that we do not provide pregnancy care, administer childhood vaccines, or perform surgeries that go deeper than the skin. We also do not carry any  oral pain medication or controlled substances in our clinic. We can provide referrals to other doctors and services for these needs.

Become a Patient

Want to become a patient? You'll need to fill out the new patient paperwork and pay the enrollment fee of $100 (or a maximum enrollment fee of $150 for families enrolling at the same time).

*Payment of monthly membership fees is required to remain an active member and receive service.

 

Want to Cancel? If you decide to cancel- it's simple. We hope to have an honest relationship with you. THERE ARE NO LONG-TERM CONTRACTS OR COMMITMENTS. We ask that you give us a 30-day notice if you wish to cancel. If you choose to become a member again, you will need to pay the enrollment fee again.
