You will be charged a 1-time Enrollment Fee of ($100 + cost of 1st month membership) or for Enrolling a family ($150 + cost of 1st month membership) [credit/bank card required].  Once your information is received, we will be in contact shortly to schedule first appointment.

Please review patient contract and forms before enrolling, if possible, print and complete them to bring to your 1st appointment.
